Elbow erroded out?

That's funny! However, without the weep hole the elbow may have continued to function indefinitely. The design, coincidentally, is similar to commercial erosion resistant elbows.

Weep hole for this kind of pipe leg support with pad is common practice. Check your company standard. If you don't see it, talk to your boss. It's original purpose is to vent the air during welding and drain condensation built up inside.
 
Yes. That's the company standard.
Here it is draining quite a lot of condensate.

Looking at the algae bloom on the right side of the picture, it would appear that this fitting had been on its way out for quite a while.
