I have always wondered what the solution is, when you find out there was no preheat applied to base material and the thickness does not call for PWHT?

Not much you can do other than hardness testing and NDT to evaluate if the lack of preheat caused a problem either during or after welding. It also depends on the materials being welded.
 
If you want an Authorize Inspector to sign off on that job, you should also make a non-conformance report and submit it to the buyer.
 
Code 'non-mandatory' appendices are only so until a lawyer gets involved.

Mechanical testing will probably not be definitive unless the violation was egregious. In any event, the extent of such testing will be very limited unless you wish to start sectioning the product.

Initiate an NCR and consult your AI.
