How did Pipe Efficiency Come About?

Although I have used pipe efficiency in the past, I was wondering how it came to be in equations? Was it Panhandle? What does the efficiency really measure?

In my opinion it came about using a dry gas formula when there was some amount of condensation taking place in the line.
