Epoxy Rock Bolt Failure

Besides the I-90 tunnel, does anyone know of any cases (small or large) where there was a failure associated with the use of epoxy rock/tension bolts? I am looking for leads here, so anything helps!

i guess you're looking failures that resulted in death and/or litigation? not the sort of failure that resulted in re-installing the bolts during normal construction activity?
