It appears to me that IV-A and VI-A are the same. My grand parents in Walla Walla got converted to touch-tone dialing very early on as a pilot test location. Their phones were all of the IV-A/VI-A configuration. It was much later before I saw a phone with 12 buttons.
 
Well there are 18 layouts shown and the title of the article says 17, so I guess the duplication of IV-A and VI-A wasn't, really (but then II-A and VI-B are also the same).
