CSB released the final report the Ethylene Release and Fire at Kuraray America, Inc. EVAL Plant.
One of the contributing factors that made this incident worse was that the PSV had a tail pipe that was installed in a horizontal position which ended up released flammable vapor towards an ignition source.
I have seen many PSVs installed like this at refineries unfortunately, so I want to raise awareness.
