Formulas to Design Thrust Blocks

I know... I know. To design a Thrust Block I have to use the momentum formula and take in account the weight of the elbow itself and the water inside it if it's a vertical thrust block, but I am a little confused with the procedure.

Regarding my last "THREAD", the engineer multiplied the working pressure (150psi) by two to get "P", and then, as the formula indicates, T=2PAsin(phi), he multiplied "P" by two; so, actually, he multiplied the working pressure by four without, really, taking in consideration the momentum. Do I am considering something not supposed to be considered in the design of the thrust block? I'll be grateful if someone of you show me an example on how to design a vertical and horizontal thrust block.
