Design of FM Antenna pole mount

I was wondering if anyone would know of any good sources or examples of how to design a pole to mount an fm antenna on top of a guyed tower or self supporter. Also, I am looking for examples of foundation designs for pad and pier and also caisson design. Any information would be of great help.
