Obtain a copy of the "Guidelines for Hazard Evaluation Procedures" published by the Center for Chemical Process Safety of the American Institute of Chemical Engineers.

You may be able to order it online from this website:

Those Guideline are pretty much the "bible" on HAZOPS and Risk Analyses for the chemical industries in the United States.
