Base Plate Leveling Pads

Can anyone point me in the direction of a good source of online information on constructing grout pads for leveling of equipment? Looking for construction methods and grout specs.

I am installing several 4,000 gallon holding tanks (each on 4 legs with 8"x8" base plates) on a sloped concrete floor at a water treatment plant. Since my tanks are connected in series they must be level to each other. Due to the slope of the floor the base plates range from 0.5" to 4" off the floor.

Looking for a source of info dispcussing max thickness of the base plate and how much wider the pad must be compared t the thhickness of the pad.
