    P91 welding requires great care. Lack of penetration is a serious problem. No inspection was performed during the root pass of the weld, and I assume the preheating and argon purging weren't inspected either. What else happened that you don't know about?

    This is due to a lack of inspection at the root of the weld. The welder has failed and a retest is required. The defective weld is not approved.
