I mean that if N2 filled bladder rupture than that N2 will be in mixed with Oil so that pressure fluctuation will take place. that was my concern. i am not sure for that scenario so would like to know that.
what happen if accumulator bladder failed during running system as filled N2 will go to system. so pressure get drop suddenly/ disturb or what will the consequences ?

No assembly mistake, it was installed as per drawing. notch matching done. Leak is very minor I had doubt of oil supplying to piston rod tubing which supplying oil spray injection.

Hot Bolting considering with risk as working required with live line during hot bolting
This flange is 10" 300# so we are using manual tightening technique not as per torque.
After leakage flange torque not checked but flange tighten again & leak was arrested.
