To address a few of the questions, the 4.02 bore came 4" SCH40 weld neck flanges that I was originally using. I recently discovered long weld neck flanges and maybe I could take a 4" LWNF...
Located in the US. Pipe and flanges are readily available here. Using standard weld neck flagnes, I'd have to weld bosses on to the tapered part of the weld neck in order to get the proper instrument spacing within the same overall length. Not sure if that is allowed.

I ended up switching to specifying a weld neck reducing 18x6 flange and a lap joint flange with the bellmouth welded to the interior side of the flange. Cuts the number of welds from 5 to 2 and eliminates the question of how to properly weld the SO flange.

In my head I was stuck in piping mode and I didn't think of the 18" being the boundary between B31 and BPVC. I'll look through VIII-1 discussion on welding nozzles, perhaps UG-39 holds the key...

I'm in Pennsylvania. I think the designers are starting to relax on requirements and allow flanges welded to a B16.9 (butt weld) reducer so hopefully this problem will go away soon!
