As noted it is not a code requirement but strongly recommend you include the MTR with your PQR.
I do not know any Clients or Clients Representatives (myself included) who will accept a PQR without confirmation of the mechanical and chemical properties of the test coupon.

However, consider what I noted previously.
If you are getting root repairs on welds visually accessible internally then what about the rest of the welds in that "lot" ?
If it is 100% NDT then no problem.
If it is 5%, 10%, 20% and you have radiographed...
If you can access to reweld it is obvious you can visually examine.
What about all the joints without visual access or no NDT ?
Suggest an investigation of welder / s responsible would be a good idea.

As R6155 stated if the test plate has defects what do you do about the vessel weld ?
Seems a very weird scenario.
If you were doing test plates as run off plates it is common sense that the whole plate must be code compliant (NDT) before performing mechanical tests.
